AMH Anticoagulation Therapy Unit

The Anticoagulation Clinic is staffed by licensed AMH pharmicists. The clinic is located in the education wing of AMH across from Partners in Care and the Hospital Gift Shop.

The AMH Anticoagulation Therapy Unit has been designed to assist physicians in the management of patients prescribed warfarin therapy. Patients that have been referred to the clinic by their physician, may come into the clinic to have a finger stick lab test, to determine their INR, dosing adjustment, as well as education.

Once a patient has been referred to the clinic by their physician, the patient will be contacted by the clinic to schedule their first appointment. All follow-up appointments are scheduled directly between the patient and the Anticoagulation Therapy Clinic. AMH Anticoagulation Therapy Unit is currently managing patients every on every Monday, Tuesday, Wednesday, and Thursday. We have also expanded to include a Berne site to better meet patient needs. In-Patient: 7:00 a.m. - 7:00 p.m.
Out-Patient 8:00 a.m. - 5:30 p.m.

Saturday and Sunday

In-Patient: 7:00 a.m. - 3:30 p.m.
Out-Patient: 9:00 a.m. - 1:00 p.m.

Services Provided

From sterile IV bags to oral medications, The AMH Pharmacy provides prescribed medications to our patients.
All patients at AMH have their medication regimens reviewed by a licensed pharmacist. OUr pharmacists review for possible drug interactions, allergies, and medication side effects.
A pharmacist is always available, during business hours, to answer any drug information or dosing questions.
Our clinical pharmacist is involved with in-patient medication review, drug therapy monitoring, and patient/staff education.
Pharmacy services are also provided to Woodcrest Nursing Home, The Village of Heritage, Corporate Health Services, and to our employees.
