Coping with an illness and hospitalization can be a difficult and confusing time. During a hospital stay, decisions and sorting out information can become overwhelming. The Patient Care Services of Adams Memorial Hospital & the Extended Care Unit assists patients/residents and families with the concerns and challenges of the patient’s/resident’s illness and hospitalization.
The PCS staff is dedicated to providing individual care to our patients/residents and their families. We interact as part of the professional hospital team to improve the quality of life of each patient.

Patient Care Services work as discharge planners to help access and coordinate additional health services. In collaboration with the healthcare team, a discharge plan is developed to manage health needs when the patient/resident leaves the hospital. During the hospitalization, options discussed may include:
